SPOILER
Regulus Black was the younger brother of Sirius Black. He joined the Death Eaters during Voldemort's first rise. In Harry Potter and the Order of the Phoenix, Sirius Black says that Regulus was killed by order of Voldemort in 1980 when he tried to leave the Death Eaters; others say that in so stating, Sirius was being an unreliable narrator.
Some theorists believe that Regulus is alive. Many others believe that Regulus Black is "RAB", the person who took the Horcrux that Dumbledore and Harry tried to obtain at the end of Harry Potter and the Half Blood Prince. This theory is supported by an edit in the summer of 2005 to The Lexicon, which said that an official Half-Blood Prince translator had been told that Regulus's middle name was Arcturus. Also, as of November 19, 2005, with the release of the Dutch version of HBP, this theory gets additional support from the translation of the initials on the note. In that series, the last name of the Black family is, and always has been, Zwarts.
Then, in the seventh book you find out that he is RAB, and that he wasn't killed on Voldemort's orders. He died because he went to the island (where Dumbledore took Harry in the sixth book) with Kreacher, where the Horocux (locket) was, ordered Kreacher to get it once he (Regulus) had drunk the potion. Unfortunately, he was dragged under water by Inferi. In addition, the theory of Regulus' middle name is confirmed by the sign on his bedroom door at Number 12, Grimmauld Place. Arcturus is the name of a star, as is Bellatrix, Sirius, Regulus, Andromeda, and several other Black family names.
